Warranty Request

The following list should be provided to the warranty department no earlier than thirty days (30) from closing. This form may be faxed to 817-439-3186, e-mailed to or mailed to our corporate office at the address above. Please do not turn in warranty forms to any other sales office or give them to any Maverick employee in the field. All warranty work will be handled Monday through Friday between 8:00 am and 5:00 pm and someone will need to be home during the warranty times scheduled.
